To schedule your visit, you'll just need a recommendation form completed by your healthcare provider (doctor, pediatrician, midwife, or nurse practitioner). Download the form below and send it to your provider through their patient portal. Once it's completed and returned, I'll be happy to help you get your visit scheduled.
If your insurance is not listed, self-pay options are available. A superbill will be provided for you to submit to your insurance company for possible out-of-network reimbursement.
